Palliative Care

We will assist you in living as fully and as comfortably as possible with a life limiting illness. Our highly skilled staff are here to ensure your end of life plan is adhered to. We will work collaboratively with your medical team to assist with all of your palliative care needs.
Training and assessment

We offer individual and group training and assessment for disability support workers, participants and NDIS staff, providing complex care in the following areas:
Bowel Care
Stoma management
Dysphagia
Superapubic catherters
Diabetes
Manual handling
Mental health
Wound care
Enteral feeding
Sensorary impairment
Epilepsy
Complex care plan development
Innovative community participation

We will assist participants to access main stream activities that they enjoy. This can be anything from football games, fishing trips, bowling, shopping trips, community events and holidays. We are here to support you in having a unique experience
Household tasks

We will assist participants to maintain their general area of living including:
• Cleaning
• Gardening
• Basic indoor & outdoor home maintenance
• Laundry/Linen
• Meal preparation
Specialised support employment and higher education
-
We will assist you in finding and maintaining employment. We can develop your resume, assist with interview preparation and attend and supervise in the workplace. We also offer assistance with accessing higher education based upon your passion and ability. We are committed to assisting you to set and achieve your goals while we support you through the process.
Assistance in coordinating or managing life stages, transition and supports
-
We will assist our participants capacity to manage different aspects of life such as transitioning through different milestones, individual skill development as well as navigation through the complexity of the NDIS. Providers may be required to provide services such as peer support, mentoring, mental health recovery and coaching.
Community nursing care
-
We offer NDIS participants living with complex conditions a comprehensive, individualised and outcome orientated approach. Our approach is to develop complex care plans focused on minimising risk while improving the quality of life for those we serve.
Development of daily care and life skills
-
We will assist you to develop and maintain essential life skills.
-learning and using public transport
-Resume writing and interview preparation
- Social skills and attending social event such as shopping trips, leisure activities, community events and appointments
-Physical activity skills
Participation in the community, social and civic activities

We will assist participants to attend areas in the community. For example shopping trips, appointments, leisure activities.
Aged Care
-
We will provide you with assistance to live a fulfilling life, we will assist you with personal hygiene, respite in the home and daily support to engage in community events and activities. Our highly trained staff can administer medication, attend to wounds care, vital signs, assessment and care planning.
Assistance with daily personal living and travel arrangements
-
We will provide assistance and supervision of personal tasks of day to day living to enable participants to live as autonomously as possible. Ut mostly consists of support with hygiene, dressing, communications and lifestyle support. In short terms, it is about providing general personal care and support.
We will assist participants with travel/transportation to and from destinations.
Assistance with daily life tasks in a group or shared living arrangement
-
We will help participants receive care and support within a variety of different shared living arrangements such as SIL,STA,MTA,ILO. It is designed to try an allow participants to live as independetely and autonomously as possible whilst still being in a safe and supportive enviroment.
The care provided will include all aspects of personal care such as hygiene requirments and other day to day supports that participants require regularly.
Group and centre-based activities

We will take participants to activities that are within a group or centre based which allow the participant to have enjoyment in various activities or events.
Therapeutic supports

We will assist participants to gain access to various support services that are beneficial and therapeutic to participants.
Examples include:

Art therapy
Alternative therapies.
